  • *National Grid requires the following form be completed and submitted to verify that there is adequate gas supply from the street to the property. Please select all new and existing gas appliances at the property.

  • *PLEASE NOTE THAT NO WORK CAN BE DONE UNTIL NATIONAL GRID VERIFIES THAT THERE IS ADEQUATE GAS SUPPLY TO ADD THE NEW APPLIANCE(S).

 

Step 4: Important Information Needed To File Your Permit

  • We require proof of your license and insurances for each specific Village/Town. Please note that the appropriate Village/Town must be listed as the Certificate Holder & Additionally Insured. We will not be able to file your permit without this information.
  • All surveys must be legible and full size. (Generators must be clearly marked with distances to all lot lines as well as the distances from the house.) We will not be able to file your permit without this information.
  • Additional Documents: Checks to village/town for permit fee; contractor/electrician license and insurances naming the town/village as the certificate holder (some request to be additional insured); tax bills, copies of COs, specifications, customer signatures, etc. The customer is responsible for calling in final building inspection if required.
